New ID card and address for MetLife Dental claims
MetLife has a new address for submitting
dental claims. The MetLife dental claim form and the generic dental ID card
have been updated with the new address.
The new address is:
MetLife Dental Claims
P.O. Box 981282
El Paso, TX 79998-1282
(All dental expense claims must be
postmarked to MetLife no later than June 30th following the end of the
calendar year in which you had the expenses, otherwise, they will be denied
as untimely.)
Dental ID cards:
The generic dental ID card can be found
here. Participants should print a copy of the new card for their
records.
Dental claim forms:
The MetLife Dental claim forms can be found on the Your Benefits Resources (YBR)
Web site under the "Contact Us" section. There is also a link to claim forms
on the
BCBS Claim forms Web site.

health benefit coverage, you can call your provider directly. You can also
call the TI Benefits Center. Both can be accessed through
